Program Description

TennCare CHOICES is for adults aged 21 and older with a physical disability as well as seniors aged 65 and older. CHOICES offers Home and Community Based Services (HCBS) in the home, on the job, or in the community to assist with daily living activities and allow people to work and be actively involved in their local community. CHOICES also provides care in a nursing home if this is needed. Through improved coordination of care and use of more cost-effective home and community-based alternatives, CHOICES expands access to home and community-based services for persons who are “at risk” of needing nursing facility services, thereby delaying or preventing the need for more expensive institutional care.
