Program Description
The National Cardiovascular Health Program is focused on cardiovascular disease (CVD) prevention and management. The program tracks and monitors the quality of care provided for those at highest risk of CVD, and provides team-based care that support community referrals, self-management, and lifestyle changes.
Before 2023, the program was a combined cooperative agreement with Diabetes that was established in 2013.
