Program Description

Provides support to children who have intellectual and developmental disabilities and are in state custody. Support can include direct care services, support for foster families, and home and community-based services for children with complex needs. Direct care includes short-term placement in community homes for children with high medical needs currently awaiting placement while receiving care in a hospital/emergency room.

Logic Model

If we provide short-term stable placements to children with disabilities and complex medical needs in state custody to transition them out of an institutional setting, then these children can receive care in a community-based setting where they can stabilize care and therapies and identify appropriate foster placements, ultimately achieving permanency and living the life they choose.
